Endoscopic Sleeve Gastroplasty(ESG) Procedure

What’s Endoscopic Sleeve Gastroplasty?

Endoscopic Sleeve Gastroplasty (ESG) is a minimally invasive, outpatient, non-surgical, incision-less weight loss procedure. The endoscopist sutures the stomach from the inside to reduce its size, which results in long-lasting weight loss. The procedure is very well tolerated. The patients are not admitted to the hospital, and they return to their normal routine after a few days.

Who is ESG for?

ESG is most suited for patients who were unable to achieve the desired weight loss with diet and lifestyle changes. A gastric sleeve is also suited for patients who do not want to undergo surgery or who do not meet the criteria for traditional bariatric surgery.

The perfect candidate for ESG is a patient with a BMI between 30 and 40 who is committed to the weight loss program and willing to adapt to a healthier lifestyle.

How is ESG performed?

The procedure is done endoscopically, so no incisions are needed. The endoscopist advances an endoscope (a tube with a camera and a light on its tip) through the mouth to the stomach. The scope is equipped with a suturing device that allows us to suture the stomach from inside. This way, we can decrease the size of the stomach by about 70-80%. The stomach can accommodate smaller meals; but also the rate by which the food empties from the stomach is decreased, so a smaller meal lasts for a much longer period of time. The patient can follow a low-caloric diet without feeling hungry all the time.

How much weight would you lose?

The transformation that takes place with this procedure can be quite remarkable. To illustrate this, consider the gastric sleeve before and after statistics: most people lose around 18 to 20 percent of their total body weight at 1 year after the procedure. Most of the weight loss occurs early on, but it is maintained. Five years after the procedure, people have maintained a 15% total body weight loss. The key is to adopt a healthier lifestyle in the long run.

Other health benefits of ESG

At The Silhouette Clinic, we understand that the journey to better health goes beyond just weight loss. That’s why our Endoscopic Sleeve Gastroplasty procedure is designed to help you lose weight and improve your overall health.

Our ESG procedure aids in significantly reducing weight, with patients typically losing more than 16% of their total body weight. However, the benefits don’t stop there. Studies have shown that ESG has a transformative effect on various health indicators. It has been observed to lower blood sugar, systolic blood pressure, and triglyceride levels within just 12 months of the procedure.

ESG is also particularly beneficial for patients with high initial Body Mass Index (BMI) and physically inactive patients. Our patients have seen marked improvements in their health, with some studies even indicating complete remission of diabetes in some patients.

Moreover, ESG demonstrates effectiveness in managing obesity-related liver conditions. Research indicates that the risk of non-alcoholic fatty liver disease, insulin resistance, and insulin and triglyceride levels can also be improved after undergoing the ESG procedure.

But at The Silhouette Clinic, we believe that a fulfilling life is about more than just physical health. That’s why we’re particularly proud that ESG contributes to weight loss and enhances the patient’s quality of life and physical activity levels. Here at The Silhouette Clinic, we don’t just aim for weight loss; we aim for wellness.

What to Expect

At The Silhouette Clinic, we strive to provide comprehensive guidance and support throughout your weight loss journey. Before you undergo the Endoscopic Sleeve Gastroplasty, our dedicated doctors will thoroughly explain the preparatory steps and procedures. You may be required to conduct some lab tests, fast from midnight the day before the procedure, and alter your medication schedule.

The ESG procedure itself, lasting around 45 minutes, is performed while you are under general anesthesia for maximum comfort. We employ an endoscope, a flexible device equipped with a light, camera, and suturing equipment, to perform the operation internally without any external incisions. Your doctor will skillfully place and tighten sutures in your stomach, reducing its volume by about 70%. This transformed structure, similar to a sleeve, enables you to feel full after eating less, absorb fewer calories, and ultimately lose weight.

Post-procedure, all our patients are able to go home the same day. Please ensure someone can safely drive you home upon discharge.

Your journey with The Silhouette Clinic doesn’t end after the procedure. We ensure you receive comprehensive dietary guidance to help your body adjust to the reduced stomach volume and sustain healthy weight loss. Your initial recovery period will consist of a gradual re-introduction of various food textures, starting with clear liquids and slowly progressing towards small portions of solid food.

To ensure long-term success, we strongly recommend adopting a lifestyle of mindful eating. This includes eating slowly, chewing your food well, staying hydrated, and stopping when you feel full. We encourage a focus on small portions of low-sugar, low-fat, and low-calorie foods. Our nutrition experts will be available to provide further guidance on healthy eating habits, empowering you to embrace a healthier, happier lifestyle.

ESG FAQs

What is ESG?

Endoscopic Sleeve Gastroplasty (ESG) is a minimally invasive, outpatient, non-surgical, incision-less weight loss procedure. The endoscopist sutures the stomach from the inside and invaginates it on itself to reduce its size, which results in long-lasting weight loss.

How is ESG different than laparoscopic sleeve gastrectomy?

ESG is NOT surgery. A non-invasive gastric sleeve procedure is performed endoscopically through the mouth, so there are no incisions and no scars. While surgical sleeve gastrectomy requires admission to the hospital for 1-2 days, ESG is an outpatient procedure. You are monitored in recovery for a couple of hours then you go home the same day. We do not cut or remove any part of the stomach, so undergoing an endoscopic gastric sleeve procedure is a much safer option than surgery.

Is ESG reversible?

We usually do not reverse the ESG. It is a permanent procedure. The stomach remains in the same shape long term. This is key to better results and long-term maintained weight loss. However, if needed, we could remove the sutures endoscopically and reverse the ESG procedure.

Do the sutures need to be removed?

No. Initially, the sutures hold the stomach in a sleeve shape. When the stomach heals, it will remain in its new shape and size long term, and the sutures are no longer needed. However, we do not remove the sutures. The sutures are very small and thin, and they remain within the wall of the stomach, and you will not feel that they are there.

What are the most common side effects?

The most common endoscopic sleeve gastroplasty side effects most patients will notice is a feeling of nausea and some abdominal pain or discomfort after the procedure. These symptoms are usually mild and can be controlled with medications. The symptoms usually resolve in a few days.

Am I a candidate for ESG?

ESG is most suited for patients who were unable to achieve the desired weight loss with diet and lifestyle changes.

While there is no strict BMI requirement for the procedure, the perfect candidate for ESG is a patient with a BMI between 30 and 40 who is committed to the weight loss program and willing to adapt to a healthier lifestyle.

ESG is also suited for patients with higher BMIs who do not want to undergo surgery.

Are there any contraindications for the procedure?

Conditions such as previous stomach surgeries and current gastric ulcers are contraindications for the procedure. Major medical problems such as advanced liver disease can also be contraindications.

How long does the procedure take?

An ESG procedure takes around 45 minutes. You will be asked to present to the endoscopy suite an hour before the procedure. After the procedure, we will watch you in the recovery area for a couple of hours. We will give you additional IV fluids and any necessary pain or nausea medications, then you will be discharged home.

When can I return to work?

After undergoing the non-surgical gastric sleeve procedure, your recovery period at home will typically span 2-3 days. Following this brief recuperation, you can confidently return to work and resume your regular routine.

How much weight can I lose with ESG?

ESG weight loss is typically quite impressive as most people lose around 18 to 20 percent of their total body weight at 1 year after the procedure. The weight loss is maintained, and at 5 years after the procedure, people have maintained a 15% total body weight loss. Weight loss can vary depending on the patient’s motivation and adherence to the dietary recommendations. The key is to adopt a healthier lifestyle in the long run.

What diet and lifestyle changes are recommended after ESG?

Following an ESG procedure, patients are encouraged to follow a long-term, low-calorie diet that is high in protein. This balanced way of eating, coupled with consistent regular exercise, is crucial to help reduce body weight. Regular follow-ups with our medical team are an integral part of the journey, aimed at supporting and monitoring the weight loss progress initiated by the ESG procedure and maintaining it over an extended period.
